Abstract

Implementing electronic medical records (EMR) is an effort to improve the efficiency of healthcare services. However, challenges such as inadequate infrastructure, unprepared human resources, limited funding, lack of system usage clarity, and absence of effective monitoring and evaluation mechanisms often hinder optimal implementation. These factors collectively impact the successful adoption of EMR systems in healthcare facilities. To identify management elements (man, material, method, machine, money), as well as planning, implementation, monitoring and evaluation, and follow-up in the implementation of electronic medical records at the Regional Mental Hospital of Lampung Province. This study used a qualitative approach with key informants including the Head of Administration and Head of Medical Records. Additional key informants were the Head of IT, IT staff, specialist doctors, nurses, pharmacists, laboratory analysts, and medical records staff. Supporting informants were drawn from non-medical hospital personnel. Data were collected in May 2025 through in-depth interviews, observation, and review of supporting documents. Data validation was conducted using triangulation of sources, techniques, and time. The hospital has not yet implemented an electronic medical record system, nor does it have standard operational procedures for EMR. Existing infrastructure, such as network systems, computers, and servers, is insufficient. The national budget efficiency policy has impacted the hospital’s ability to provide supporting infrastructure. During implementation attempts, obstacles included low user competence and unstable internet connections leading to system downtime. Evaluation processes have not yet been conducted. The implementation of EMR at the Regional Mental Hospital of Lampung Province remains suboptimal due to infrastructure limitations, lack of SOPs, and inadequate user readiness. It is recommended to improve internet bandwidth through collaboration with service providers and to develop an offline-based EMR system supported by local server storage in partnership with an information technology university
