Abstract

Perilaku kekerasan merupakan salah satu kondisi kegawatdaruratan psikiatri yang memerlukan identifikasi dan penanganan cepat untuk mencegah terjadinya cedera pada pasien, keluarga, maupun tenaga kesehatan. Australasian Triage Scale (ATS) digunakan sebagai sistem triase untuk menentukan prioritas pelayanan berdasarkan tingkat kegawatan pasien. Penelitian ini bertujuan menggambarkan implementasi ATS pada pasien risiko perilaku kekerasan di instalasi gawat darurat psikiatri. Penelitian menggunakan desain deskriptif dengan pendekatan studi kasus pada 13 pasien yang mengalami risiko perilaku kekerasan. Data diperoleh melalui observasi, wawancara keluarga, dan dokumentasi hasil pengkajian triase. Data dianalisis secara deskriptif menggunakan analisis kasus (case analysis) dengan mengelompokkan data berdasarkan kategori Australasian Triage Scale (ATS), karakteristik klinis pasien, tingkat risiko perilaku kekerasan, dan intervensi yang diberikan, kemudian membandingkan hasilnya dengan pedoman ATS untuk mengevaluasi ketepatan penentuan prioritas pelayanan. Hasil penelitian menunjukkan bahwa sebagian besar pasien berada pada kategori ATS 1 (46,2%), diikuti ATS 2 (23,1%), ATS 3 (15,4%), dan ATS 4 (15,4%). Manifestasi klinis yang ditemukan meliputi agitasi, perilaku agresif verbal dan fisik, ancaman kekerasan, halusinasi perintah, perilaku melukai diri sendiri, serta ide bunuh diri. Implementasi ATS membantu perawat mengidentifikasi tingkat kegawatan secara cepat dan menentukan prioritas intervensi keperawatan berupa observasi ketat, pengamanan lingkungan, komunikasi terapeutik, restrain sesuai indikasi, dan terapi psikofarmakologis. ATS terbukti mendukung pengambilan keputusan klinis serta meningkatkan keselamatan pasien dan tenaga kesehatan dalam pelayanan kegawatdaruratan psikiatri.
