Determinan Hipertensi Intradialitik pada Pasien Penyakit Ginjal Kronis: Tinjauan Literatur Terhadap Faktor Usia dan UFR

Abstract

Hemodialisis memiliki risiko komplikasi hemodinamik yang perlu diperhatikan, salah satunya adalah tekanan darah intradialitik.  Tujuan penelitian ini adalah determinan hipertensi intradialitik pada pasien penyakit ginjal kronis ditinjau berdasarkan faktor usia dan ultrafiltration rate (UFR). Metode yang digunakan adalah penelusuran literatur 10 tahun terakhir, pada database google scholar dan Pubmed dengan kata kunci hemodialysis, intradialytic dan Ultrafiltration Rate. Artikel yang didapatkan sebanyak 34 artikel dan 12 artikel yang memenuhi syarat. Telaah dilakukan dengan cara membuat resume dan kesimpulan dari penelitian. Hasil penelusuran artikel didapatkan bahwa Diagnosa Hipertensi Intradialitik (HID) melalui pemantauan tekanan darah pre dan pasca-hemodialisis adalah metode utama untuk menilai komplikasi hemodinamik pada pasien Penyakit Ginjal Kronis (PGK), yang biasanya dilaporkan sebagai peningkatan tekanan darah sistolik atau Mean Arterial Pressure (MAP) menggunakan cut-off tertentu, baik 10 mmHg maupun >15 mmHg. Kejadian ini sering dikaitkan dengan variabel klinis seperti usia, status diabetes, dan durasi hemodialisis, serta parameter teknis seperti Ultrafiltration Rate (UFR), Interdialytic Weight Gain (IDWG), dan kepatuhan terhadap pembatasan cairan.
