Application of 432 Hz Frequency Music Therapy on Changes in Anxiety Levels in Pre-Operating Hernia Patients: A Case Report

Abstract

Preoperative anxiety is a common psychological response among surgical patients and may be associated with physiological changes, including increased blood pressure, pulse rate, and respiratory rate. Music therapy has been used as a non-pharmacological nursing intervention to promote relaxation; however, evidence on the use of 432 Hz music therapy in preoperative hernia patients remains limited. This case report aimed to describe changes in anxiety levels, information needs, and vital signs following 432 Hz music therapy in a preoperative patient with an inguinal hernia. This study used a descriptive case report design involving one 63-year-old male patient diagnosed with a left lateral inguinal hernia who experienced preoperative anxiety. Data were collected through interviews, observations, vital sign measurements, and medical record reviews. Anxiety and information needs were assessed before and after the intervention using the Amsterdam Preoperative Anxiety and Information Scale (APAIS). The intervention consisted of 10 minutes of instrumental music therapy tuned to 432 Hz in the preoperative room. The data were analyzed descriptively to identify changes in anxiety levels, information needs, and physiological parameters before and after the intervention. After the intervention, the APAIS anxiety score decreased from 18 to 13, although it remained within the preoperative anxiety category. The APAIS information needs score decreased from 8 to 6, indicating a shift from high to moderate information needs. Physiological changes were also observed, including decreases in blood pressure from 170/80 mmHg to 136/84 mmHg, pulse rate from 100 to 92 beats per minute, and respiratory rate from 22 to 18 breaths per minute. The application of 432 Hz music therapy was associated with decreased anxiety scores, information needs, and several vital sign parameters in this preoperative hernia patient. However, because this report involved a single case without a comparison group, the findings should be interpreted cautiously and cannot be generalised.
