Rosemary Aroma and Warm Therapy for the Management of Primary Dysmenorrhea: A Comfort-Based Approach among Midwifery Students

Abstract

Adolescence is a crucial transitional stage characterized by reproductive system maturation and complex hormonal changes. One of the effects of these changes is primary dysmenorrhea, a common condition among adolescent females worldwide, including in Indonesia, with a prevalence ranging from 50% to 90%. This condition is caused by increased prostaglandin production, resulting in excessive uterine contractions, vasoconstriction, and ischemia, which lead to severe lower abdominal pain accompanied by nausea, fatigue, and reduced daily activity. This study aimed to apply warm compresses combined with rosemary essential oil as a non-pharmacological intervention to alleviate primary dysmenorrhea among midwifery students at Yatsi Madani University. The study employed a quasi-experimental design using a pre-test and post-test control group approach. The sample was selected using accidental sampling, with 16 students with dysmenorrhea in each group, resulting in a total of 32 participants. The intervention group received rosemary-aroma warm compresses based on a standard operating procedure. Pain intensity was measured using the Visual Analogue Scale (VAS). Data were analyzed using the non-parametric Mann–Whitney test to determine differences between the control and intervention groups. The application of warm compresses with rosemary essential oil showed a decrease in dysmenorrhea pain intensity scores compared to the control group, indicating that the combination therapy effectively reduced menstrual pain levels by p-value 0.001. Warm compress therapy combined with rosemary essential oil can serve as an effective, simple, and safe non-pharmacological intervention for managing primary dysmenorrhea among young women. This approach not only reduces pain intensity but also improves comfort and daily functioning.
